Outpatient Registered Nurse- Patient Aligned Care Team

Summary

The Cincinnati VA Medical Center is recruiting for a Patient Aligned Care Team (PACT) Registered Nurse (RN) at its Georgetown - OH Community Based Outpatient Clinic (CBOC). The PACT RN executes responsibilities that demonstrate leadership, experience and creative approaches to management of complex primary care.

• English Language Proficiency:
  Proficiency in basic written and spoken English is required.
• Graduate of an approved nursing school accredited by ACEN or CCNE.
• Or individuals in a masters‑level bridge program with equivalent coursework may become registered as a nurse with a state licensing board before completing the bridge program.
• Upon achieving a state license, appointment may be temporary and later converted to permanent upon completion of the bridge program.
• Foreign graduates with current, full, active and unrestricted registration meet the requirement; credit for higher education requires formal equivalency validation.
• Graduate Nurse Technician Exception: candidates meeting education but not licensure may be appointed as GNT on a temporary appointment while pursuing licensure.
• Grade determination scope‑education criteria apply (details available in online assessment).
• Preferred experience:
  
  two years of primary care experience.
• Physical requirements:
  
  standing, walking, stooping, bending, reaching, lifting; capable of moving clients up to 50 pounds and using mechanical devices; visual and auditory acuity necessary; ability to control or defend against emotionally ill clients.

• Provide gender‑specific direct veteran care and care coordination within the Primary Care Department.
• Manage assigned patient panels, utilize registries, and ensure appropriate access and evaluation.
• Provide health promotion, health education and coaching on wellness, disease prevention and chronic care management.
• Collaborate in development, implementation, evaluation and revision of policies, procedures and guidelines.
• Demonstrate performance and leadership that improves care for the Primary Care Department and facility.
• Provide and receive constructive feedback for improvement.
• Support colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
• Foster a safe and supportive environment conducive to professional development.
• Evaluate outpatient outcomes of evidence‑based decisions and practice changes for individuals, groups and populations.
