Summary
The Cincinnati VA Medical Center is recruiting for a Patient Aligned Care Team (PACT) Registered Nurse (RN) at its Georgetown - OH Community Based Outpatient Clinic (CBOC). The PACT RN executes responsibilities that demonstrate leadership, experience and creative approaches to management of complex primary care.
Qualifications- English Language Proficiency:
Proficiency in basic written and spoken English is required. - Graduate of an approved nursing school accredited by ACEN or CCNE.
- Or individuals in a masters‑level bridge program with equivalent coursework may become registered as a nurse with a state licensing board before completing the bridge program.
- Upon achieving a state license, appointment may be temporary and later converted to permanent upon completion of the bridge program.
- Foreign graduates with current, full, active and unrestricted registration meet the requirement; credit for higher education requires formal equivalency validation.
- Graduate Nurse Technician Exception: candidates meeting education but not licensure may be appointed as GNT on a temporary appointment while pursuing licensure.
- Grade determination scope‑education criteria apply (details available in online assessment).
- Preferred experience:
two years of primary care experience. - Physical requirements:
standing, walking, stooping, bending, reaching, lifting; capable of moving clients up to 50 pounds and using mechanical devices; visual and auditory acuity necessary; ability to control or defend against emotionally ill clients.
- Provide gender‑specific direct veteran care and care coordination within the Primary Care Department.
- Manage assigned patient panels, utilize registries, and ensure appropriate access and evaluation.
- Provide health promotion, health education and coaching on wellness, disease prevention and chronic care management.
- Collaborate in development, implementation, evaluation and revision of policies, procedures and guidelines.
- Demonstrate performance and leadership that improves care for the Primary Care Department and facility.
- Provide and receive constructive feedback for improvement.
- Support colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.
- Foster a safe and supportive environment conducive to professional development.
- Evaluate outpatient outcomes of evidence‑based decisions and practice changes for individuals, groups and populations.

LocationsThe Cincinnati VA Medical Center (CVAMC) is a two‑division campus located in Cincinnati – OH and Fort Thomas – KY serving 15 counties in Southwest OH – Northern Kentucky – and Southeast Indiana with six Community Based Outpatient Clinics located in Bellevue – KY;
Florence – KY;
Lawrenceburg – IN;
Hamilton – OH;
Clermont County – OH; and Georgetown – OH. The CVAMC also supports two off‑main‑campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
