Nurse Practitioner

Position Overview

Full‑Time Nurse Practitioner at Senior Community Care of Michigan PACE.

• Monday - Friday, 8:00 a - 4:30 p, rotating on‑call after hours.
• Fully onsite.
• Occasional weekends.

• Intermittent local travel to participants’ homes.

• Provides primary care to participants under supervision of the Medical Director.
• Performs physical assessments of new PACE enrollees and semi‑annual reassessments, and develops and implements care plans.
• Evaluates participant physical complaints and provides appropriate treatment.
• Provides teaching and education to participants and caregivers.
• Functions as a member of the Interdisciplinary Team (IDT).
• Integrates primary care treatment plan into the overall plan of care developed by the IDT.
• Conducts in‑person reassessments semiannually and as needed.
• Participates in discharge planning for acute and long‑term placement.
• Coordinates care with Primary Care Physicians and Medical Directors of VOANS skilled nursing facilities.
• Responds to emergent and acute needs of participants.

• Oversight of residents of VOANS skilled nursing facilities.
• Provides medical care and treatment for residents of VOANS skilled nursing facilities.

• Licensed Registered Nurse (R.N.) in the state of employment.
• Graduate of a professional nursing school.
• Certified as a nurse practitioner by a national certifying organization recognized by the state board of nursing.
• Minimum of one year experience working with frail and elderly population.
• Training or related experience in the assigned job.

• Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred.
• Two years experience as a nurse practitioner in a geriatric setting desirable.
