Seeking a skilled and dependable Registered Nurse to provide high quality home health care and case management services. This role involves managing a caseload of patients while delivering a full range of home health visit types to support patient recovery, independence, and overall well being in the home setting.
Schedule & Work Details:
- Location:
Lake, Ashtabula, and Geauga Counties - Schedule:
Monday through Friday, 8:00 AM to 4:30 PM - Weekly
Hours:
40 hours, with a 36 hour guarantee - On Call/Weekends: 1 week and 1 weekend per month
- Productivity Expectation: 30 units
- Pay Rate: $51 to $53 per hour
- Mileage Reimbursement:
Yes, reimbursed between patient visits
Key Responsibilities:
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Manage a patient caseload including coordination of care and ongoing case management responsibilities
- Perform all home health visit types including admissions, routine visits, recertifications, and discharges
- Conduct comprehensive assessments and monitor patient progress
- Educate patients and caregivers on disease management, medications, and safety practices
- Complete accurate and timely documentation of all visits and care provided
- Collaborate with interdisciplinary care teams to ensure continuity and quality of care
- Adjust care plans as needed to support optimal patient outcomes
Required Qualifications:
- Active Registered Nurse (RN) license in the state of Ohio
- Recent home health experience required
- Strong clinical and case management skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable transportation
Preferred Qualifications:
- Experience with Homecare Homebase (HCHB)
- Strong communication and patient education skills
- Familiarity with home health best practices and regulatory standards
