Licensed Practice Nurse (LPN)

The primary purposes of the Licensed Practice Nurse (LPN) are to provide direct patient care, patient and family teaching, and work cooperatively within the interdisciplinary health care team in maintaining standards of nursing care under the direction of the Registered Nurse (RN) and within the framework of the Ohio Nurse Practice Act.

Graduate of an accredited school of nursing. Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing. Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required. IV Certification, preferred.

Expectations Demonstrates proficiency in basic nursing service, knowledge, and skills. Demonstrates knowledge of and applies evidence-based practice. Collects comprehensive data pertinent to the patient’s health or the situation. Identifies and reports changes in patients’ status to Registered Nurse for evaluation. Documents nursing activities accurately and in a timely manner. Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment under the direction of the Registered Nurse.

Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients’ responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse. Collects specimens as ordered. Interacts with and contributes to the professional development of peers, colleagues, and students. Collaborates with patient, family, and others in the conduct of nursing practice.

LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction. Attains knowledge and competency that reflects current nursing practice and applicable policies. Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules and regulations, during nursing practice. Exhibits High Reliability Organization (HRO) tools and tones. Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.

Checks physician schedules frequently for any add-ons or updates. Inspects patient’s medical record prior to visit to insure completeness as well as making sure the results of tests are available. Rooms patients in a timely fashion making sure to identify patient and compare with chart. Obtains vital signs, height, weight, and current accurate list of medications and all pertinent information needed for the physician to assess the patient.

Performs EKG according to policy and protocol. Accurately completes medical forms and correctly completes and routes billing information to appropriate staff. (Example disability forms etc..) Copies and releases medical records according to office procedures. Documents properly and legally in patient’s chart. Correctly educates patient for all testing and medications that are ordered. Returns phone calls to patients by the end of the business day.

Notifies physician promptly of abnormal lab/test results. Restocks and cleans exam rooms after use according to protocol. Uses the EHR to accurately document and inform physicians of patient interactions. Any other duties as assigned by Practice coordinator or Manager

Hazardous Exposure Category Category I - Includes tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for spills or splashes of, or mucous membrane or skin contact with blood, body fluids or tissues are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
