Position: Case Manager - Full-Time 1st Shift
Location: Brookville

Position Summary:

Coordinates a resident’s skilled care according to physician orders and ensures communication of skilled care with the interdisciplinary team, resident and family.

Essential Functions:

  • Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others.
  • Must be able to read, write and understand the English language.
Position Responsibilities:
  • Must follow Brook Haven Policies and Procedures.
  • Must meet job related competencies as outlined in the Skills Competency Manual.
  • Pre Admission:
    Conduct on-site assessments, verify payer source, complete pre admission screening form, pharmacy and med surge quotes.
  • Admission:
    Review and initiate orders, communicate with care community personnel, greet resident and family.
  • Coordinate residents MDS’s and Care Conferences.
  • Participate in the QA process and “Nurse On-call” program.
  • Ongoing communication regarding resident goals, progress and discharge planning.
  • Assume responsibility for maintaining professional competence.
  • Oversee skilled residents care and work closely with the Office Manager.
  • Perform other duties as assigned.
Knowledge, Skills and Abilities:
  • Graduation from an accredited school of Nursing.
  • Current license as a Registered Nurse with the State Board of Nursing.
  • CPR certification recommended.
