Description

Under the direction of the Medical Director, the Nurse Practitioner provides comprehensive primary care services within the scope of practice established by the Ohio Board of Nursing, written protocols, and professional competencies. This role delivers patient-centered medical care, supports preventive health initiatives, participates in quality improvement efforts, and collaborates with interdisciplinary care teams to advance Valley View Health Centers’ mission of accessible, high-quality healthcare.

Essential Functions
Functional Area: Clinical Care Delivery

Responsibilities, Deliverables, Outcomes and Expectations:

Provide direct patient care, including assessment, diagnosis, treatment, and follow-up.

Diagnose and treat common and uncomplicated health conditions; collaborate, consult, and refer patients with complex or uncommon conditions to physicians or specialists as appropriate.

Develop and implement treatment plans for acute and chronic illnesses, including preventive care services.

Conduct comprehensive health histories, physical examinations, and diagnostic testing.

Utilize established clinical guidelines and evidence-based practices in patient care.

Incorporate preventive health strategies tailored to individual, cultural, sociological, psychological, and environmental factors.

Document all patient care accurately and timely within the electronic medical record.

Participate in after-hours and weekend call rotation as assigned.

Serve as a clinical preceptor for students as needed.

Functional Area: Quality, Compliance, and Program Support

Responsibilities, Deliverables, Outcomes and Expectations:

Assist in the development, implementation, and monitoring of clinic policies, procedures, and protocols.

Participate in quality assurance activities, including chart reviews and peer review processes.

Support ongoing quality improvement and clinical benchmark initiatives.

Assist in implementing in-service training programs and continuing education initiatives.

Attend provider meetings to review productivity, clinical outcomes, and organizational goals.

Serve as liaison with the medical community and establish relationships with external agencies and organizations.

Maintain compliance with HIPAA, safety, and regulatory requirements.

Functional Area: Service and Other Responsibilities

Participate in committee assignments as requested.

Assist in maintaining orderly clinical, office, and shared work spaces.

Perform other related duties as assigned to support VVHC and CAC operations.

Education:

Master of Science in Nursing (MSN)

Job-Related

Experience:

Experience providing primary care services within an outpatient or clinical setting preferred.

People Management

Experience:

Not required.

Licenses, Certifications, and Accreditations:

Current Ohio Advanced Practice Registered Nurse (APRN) license

DEA registration

ACLS certification

Proof of automobile insurance; insurable under agency policy

Participation in continuing medical education as required for licensure

Preferred Qualifications

Experience in a community health center or FQHC setting

Prior involvement in quality assurance or clinical education activities

Knowledge of Community Action programs and integrated care models

Working Conditions

This position operates in a clinical healthcare environment and involves regular exposure to communicable diseases, blood, and bodily fluids. The role requires standing, walking, lifting, bending, and fine manual dexterity 25–75% of the time, including assisting with patient mobility. The Nurse Practitioner must be able to work under stressful conditions, maintain emotional stability, and provide safe, effective care during irregular hours as required.

Visual and tactile acuity are necessary to monitor patient conditions and deliver medical services.
