Swanton Valley Rehabilitation and Healthcare Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our nursing team on Night Shift
.

Position OverviewAs an LPN/RN at Swanton Valley, you will provide quality nursing care to our residents while helping maintain a safe, comfortable, and resident-centered environment. Our nurses play an important role in leading the care team and ensuring residents receive the attention and clinical support they deserve.
Responsibilities- Provide direct nursing care in accordance with residents' plans of care
- Administer medications and treatments as ordered
- Complete nursing assessments and monitor changes in resident condition
- Communicate changes and concerns promptly to physicians and appropriate members of the care team
- Accurately complete required clinical documentation
- Supervise and support STNAs during the shift
- Assist with admissions, transfers, and discharges as needed
- Respond appropriately to changes in condition and emergency situations
- Maintain compliance with facility policies and applicable state and federal regulations
- Promote a positive, respectful environment for residents, families, and coworkers
- Current LPN or RN license in the State of Ohio in good standing
- Long-term care or skilled nursing experience is preferred, but not required
- Strong clinical judgment, communication, and organizational skills
- Ability to work effectively as part of an interdisciplinary team
- Dependable, compassionate, and committed to providing excellent resident care
