Carespring in Cincinnati is seeking an experienced Nurse Practitioner or Physician Assistant for full-time, on-site support in transitional care coordination. You will evaluate patients before admission, provide medical oversight, educate on medication management and disease care plans, and coordinate with physicians, nurses, and case managers to ensure smooth transitions.
Hours roughly 8:00am–5:00pm, Mon–Fri, with some day-to-day flexibility; on-call not routinely required.
