Quality Moments Behavioral Health is seeking a Senior Care Case Manager to provide compassionate support to seniors in skilled nursing facilities. The role can be filled as either part-time or full-time, with duties focused on social, emotional, and behavioral well-being.
The ideal candidate will have a background in social work or counseling, strong interpersonal skills, and a valid driver’s license to visit facilities as needed. Experience with Alzheimer’s is a plus.
