Premier Dialysis is seeking a highly motivated and independent nurse to support the onboarding and ongoing management of home dialysis patients. This role is ideal for nurses looking to supplement their income while maintaining flexibility and autonomy.

This is a performance-based, per-patient role —there are no set hourly requirements
. Your focus is simple: deliver high-quality care on your schedule and the patient's schedule.

• Onboard new home dialysis patients in their residences
• Perform routine patient assessments and clinical evaluations
• Coordinate and review lab work
• Manage medication requisitions and administration as appropriate
• Ensure accurate and timely documentation
• Communicate regularly with Lead RN regarding patient status and needs
• Support patient education and retraining as needed
• Maintain compliance with all clinical and regulatory standards

• Flexible Schedule – No hourly requirements
• 75% Travel to patient homes
• Work independently while supported by a centralized clinical leadership team
• Assignments coordinated by Lead RN

• Set rate per patient, per month
• Mileage reimbursement based on company mileage chart

• Strong pathway to full-time employment as Premier Dialysis continues rapid expansion across multiple markets
• Opportunity to take on additional responsibilities in new markets and program development

• Active RN license (multi-state preferred)
• Experience in dialysis, home health, or chronic disease management strongly preferred
• Strong organizational and communication skills
• Ability to work independently and manage a flexible schedule
• Willingness to travel to patient homes (approx. 75%)
• Reliable transportation
