The Visiting Nurse Association of Florida (VNA) is a 50-year-old home health agency that is vastly growing. We are seeking an Exceptional Registered Nurse (RN) to join our team in various Florida locations.
The Registered Nurse is a qualified professional responsible for the planning, coordination, and assurance of quality care services to patients and their families.
Responsibilities include:
- Developing work methods and procedures that facilitate the rendering of high quality care.
- Delivering professional nursing services in the client's home setting and providing case management for their primary patients.
- Monitoring the changing needs of the client and or family.
- Providing initial assessment, overseeing progress towards goals, monitoring clinical outcomes.
- Maintaining ongoing communication with patient, their families, physicians, other disciplines, Clinical Manager, and office staff.
- Participating in staff Interdisciplinary conferences.
- In conjunction with the patient and/or their representative developing, implementing, and continuously monitoring the plan of care to ensure quality of care for patient.
- Completing clinical documentation, including OASIS, evaluations, orders, and documentation of care coordination.
- Working with Physicians and Discharge Planners while promoting health and education to patient and or caregiver.
- Functioning as a liaison with the community's health-related resources including DME, oxygen, and infusions.
- Interacting with patients and family, team members, and other health care professionals.
- Participating in ongoing education, seeking opportunities for continuous personal and professional development.
As a Licensed Practical Nurse (LPN), you will work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes. You will be responsible for completing physical assessments and determine any patient home care needs, assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model.
Requirements include:
- Active Registered Nurse License or Practical Nurse License required
- Must have a minimum of 1 year Clinical Experience
- Home Health/HCHB Experience a plus
