• Outpatient-only role caring for a Medicare and Medicare Advantage senior population

Practice Environment

• Senior-focused clinic designed for connection, continuity, and comfort

• Care coordination, analytics teams, and nursing support built into the model

• Collaborative culture focused on communication, wellness, and team-based care

• Monday through Friday schedule, closed 12-1 pm for lunch, with no inpatient duties

Candidate Requirements

• MD or DO, Board Certified or Board Eligible in Internal Medicine or Family Medicine

• Commitment to providing high-quality care to a 65+ patient panel

• Strong communicator who thrives in a team-driven, value-based model

• Excellent documentation habits with an emphasis on preventive care

• Spanish-speaking ability is preferred but not required
