We are seeking a motivated Hospice Community Liaison to join our team. This role is ideal for someone who thrives in the community, enjoys connecting with healthcare professionals, and understands the importance of timely hospice care for patients and families.

This is more than a sales role, it’s an opportunity to ensure patients receive compassionate end‑of‑life care at the right time.

What You’ll Do
  • Build and maintain strong referral relationships with physicians, hospitals, home health agencies, assisted living communities, and skilled nursing facilities
  • Educate healthcare partners on hospice eligibility and services
  • Identify appropriate hospice candidates and facilitate smooth admissions
  • Conduct in‑services and community presentations
  • Collaborate closely with clinical and intake teams
  • Track referral activity and support census growth
What We’re Looking For
  • 2+ years of healthcare sales, hospice, or home health experience preferred
  • Strong understanding of hospice eligibility and Medicare guidelines
  • Excellent communication and presentation skills
  • Self‑starter who is organized and goal‑driven
  • Compassionate, professional, and relationship‑focused
