California Licensed Medical Case Worker

A company is looking for a Medical Case Worker II to assess healthcare and social needs of clients and provide case management services.

Key Responsibilities Conduct initial needs assessments and interviews with clients and their families Develop and coordinate case management plans with community health workers and mental health clinicians Facilitate out-of-home placements and connect clients with community resources

Required Qualifications Bachelor's degree from an accredited college or university Professional casework experience in interviewing, counseling, and assisting clients

Experience with complex case management and home visits Bilingual Spanish preferred but not mandatory Successful clearance of LA County Live Scan and Medical Clearance Processes
