Compensation- $80 per evaluation
- Treatment rates negotiable based on experience
- Flexible per diem scheduling

- Perform comprehensive physical therapy evaluations in the home
- Develop individualized plans of care
- Implement therapeutic interventions to improve strength, mobility, balance, and safety
- Educate patients and caregivers on exercises and fall prevention
- Collaborate with nursing, occupational therapy, speech therapy, and physicians
- Complete timely and accurate documentation
- Active Oklahoma Physical Therapist license
- Current CPR certification
- Home health experience preferred but not required
- Strong clinical judgment and time management
- Ability to work independently while communicating effectively with the team
- Reliable transportation within a 50-mile radius of Oklahoma City
