We are seeking a compassionate and skilled RN Case Manager to join our dedicated team on a full-time basis. This is a role that requires travel within the greater Oklahoma City area. The RN Case Manager will assess, plan, implement, and evaluate patient care plans to ensure quality outcomes and support each patient on their path to recovery. Responsibilities include coordinating care between disciplines, managing patient discharges, overseeing utilization of healthcare resources, and advocating for patient needs.

Strong communication with patients, families, and healthcare team members is essential to success in this role.

• Clinical Management:
  Perform initial home visits, initiate Plans of Care under physician orders, and conduct regular re-evaluations.
• OASIS Proficiency:
  Handle all OASIS documentation for admissions, transfers, re-certifications, and discharges.
• Patient Advocacy:
  Educate patients and their families on disease processes, medication management, and specialized home care techniques.
• Supervision:
  Provide onsite supervision and guidance to LVNs and HHAs to maintain clinical excellence.
• Coordination:
  Facilitate case conferences with clinicians (DON, PT, etc.) and maintain open communication with physicians regarding changes in patient status.
• Compliance:
  Adhere to all CMS and State regulations, company policies, and conflict of interest standards.

• Strong nursing knowledge and clinical experience, including proficiency in direct patient care and medical protocols
• Expertise in Case Management and Discharge Planning to support patients' transition to the next stage of care
• Skills in Utilization Management to ensure optimal use of healthcare resources
• Strong organizational, communication, and teamwork skills
• Active RN license in the state of Oklahoma
• Experience in home healthcare or acute care settings preferred
• Ability to work both independently and collaboratively in a patient-focused environment
• Ability to pass a background check and pre-employment drug screen.
• Must be willing to travel within the service area.
