Position OverviewRegistered Nurse – Hospice Care
We are seeking a licensed registered nurse to conduct initial visits, develop and manage hospice care plans, and provide patient and family support.
Duties & ResponsibilitiesPatient Care
- Complete initial, comprehensive, and ongoing assessments to determine hospice needs.
- Conduct physical assessments, document history of illnesses, and use the nursing process to develop care plans.
- Write and initiate plans of care, re‑evaluate regularly, and revise as necessary.
- Administer medications and treatments per physician plan.
- Counsel patients and families on nursing and related needs, provide instructions, and assist with activities of daily living.
- Serve as case manager, coordinating care for assigned caseload.
Communication
- Maintain accurate clinical notes regarding patient condition and care.
- Record pain/symptom management changes and outcomes.
- Communicate with physicians and community health personnel, obtaining orders as required.
- Teach patients and caregivers self‑care techniques.
- Collaborate with interdisciplinary team, attending meetings.
Additional Duties
- Participate in on‑call duties per policy.
- Ensure necessary equipment and services are available.
- Supervise ancillary personnel and delegate responsibilities.
- Engage in continuing education and quality improvement activities.
- Graduate of a National League for Nursing accredited school.
- Registered Nurse with current licensure in Oklahoma.
- Minimum two years of nursing experience, preferably in public health, home care, or hospice.
- Experience supervising hospice aides (preferred).
- Current CPR certification.
- Valid driver’s license and automobile insurance for agency travel.
- Ability to lift, transfer, or reposition patients; physical capability to walk, stand, bend, and reach.
- Strong organizational, verbal, and written communication skills.
- Self‑directed with minimal supervision.
