Hospice Registered Nurse WI

A facility near Sparta, WI is seeking a Hospice Registered Nurse to join their hospice and palliative care team!

• Full Time Permanent employment
• Schedule:
  
  Day Shift;
  Mon-Fri with weekend and holiday rotation
• Collaborates with the hospice interdisciplinary team to develop and implement a comprehensive plan of care based on patient, family, and team assessments.
• Completes functional assessments and initiates the plan of care to identify needs, manage symptoms, and establish patient/family goals.
• Provides direct nursing care, supervises care staff and volunteers, and evaluates progress using strong clinical and communication skills.

• Unrestricted RN license with the ability to practice in Wisconsin
• Must have or be willing to obtain BLS and CHPN certifications
• Previous Nursing experience required
• Experience in hospice and palliative care preferred
