Job SummaryPrimary functions are to administer nursing care for hospice patients in their place of residence and to coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency. Assumes the responsibility for, and participates in, the coordination of care.
Essential Functions- Initial and ongoing comprehensive assessments of the impact of the terminal disease on patients' physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.
- Applies specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
- Implements/develops/documents the plan of care to ensure quality and continuity of care and recommends revisions to the plan as necessary.
- Consults with and educates the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Prioritizes needs with members of the interdisciplinary team.
- Determines scope and frequency of services needed based on acuity and patient/family needs.
- Assesses the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
- Applies concepts of infection control and standard precautions to prevent contamination and transmission of disease.
- Provides clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service.
- Prepares clinical and progress notes that demonstrate progress toward established goals and ensure continuity of quality patient care delivered with appropriate documentation.
- Informs physician and other personnel of changes in the patient's needs and outcomes of intervention, evaluating patient and family response to care.
- Monitors assigned cases to ensure compliance with requirements of third-party payors.
- Demonstrates commitment and professional growth by participating in Hospice-sponsored in-service programs and maintaining or improving competency, and participates in the Hospices QAPI program.
- Promotes the Hospices philosophy and administrative policies.
- Performs on-call responsibilities and provides on-call services to patients and their families as assigned.
- Provides effective communication to patients, family, team members, and other health care professionals.
Current State license as a Registered Nurse.
Driver's LicenseCurrent driver's license.
CPR CertificationCurrent CPR Certification.
ExperienceTwo (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred.
Skills- Nursing skills as generally accepted standards of practice.
- Knowledge of state and federal regulations.
- Ability to work independently and make sound decisions.
- Strong attention to detail and organizational habits.
- Excellent verbal and written communication skills.
- Good interpersonal skills.
Reliable transportation and valid auto liability insurance.
