Kingwood Skilled Nursing & Therapy is seeking a licensed nurse (RN or LPN) to join as MDS Coordinator. You will orchestrate, direct, and schedule MDS completion including RAPs and Care Plans for residents.
Responsibilities include data collection, RAP review, and ensuring accurate MDS and Care Plan documentation in compliance with state/federal regulations. Oklahoma RN/LPN license and basic computer skills required.
