We're looking for a compassionate and dependable Licensed Practical Nurse (LPN) who enjoys providing one‑on‑one patient care in a home health setting. This is an opportunity to build meaningful relationships with patients while delivering quality nursing care that helps them recover, manage chronic conditions, and remain safely at home.

• Provide skilled nursing care under the direction of a Registered Nurse and physician
• Administer medications and treatments as ordered
• Monitor patient conditions and report changes to the supervising RN
• Reinforce patient and caregiver education on medications, treatments, and disease management
• Assist with developing and following individualized plans of care
• Complete timely and accurate clinical documentation
• Collaborate with physicians, caregivers, and the interdisciplinary care team

• Provide personalized, one‑on‑one patient care

• Active LPN/LVN license (state of practice)
• Previous home health, hospice, long-term care, or clinical nursing experience preferred

• Ability to work independently while managing multiple patients
• Valid driver's license and reliable transportation
