Responsibilities

• Provide direct patient care within scope of practice, including assessments, monitoring, and clinical interventions.
• Accurately document patient encounters, interventions, and outcomes in the electronic health record.
• Assist physicians and advanced practice providers with examinations, procedures, and treatment plans.
• Administer medications and therapies as permitted by licensure and organizational policy, monitoring for effectiveness and safety.
• Collaborate with interdisciplinary team members to develop, implement, and evaluate patient-centered care plans.
• Educate patients and families on diagnoses, treatments, medications, and self-care strategies to support recovery and long-term health.
• Adhere to infection control, safety, and quality standards to maintain a safe clinical environment.
• Participate in team huddles, case reviews, and quality-improvement efforts to enhance patient outcomes and workflow efficiency.
• Support care transitions, including discharge planning and coordination with community resources when appropriate.
• Maintain professional development through training, in-services, and ongoing education aligned with Mercy's expectations.
