Job Summary:
The Hospice LPN Clinical Scheduler is responsible for coordinating daily clinical operations by managing schedules for the interdisciplinary team, ensuring patients receive timely, appropriate care in accordance with physician orders, hospice regulations, and organizational standards. This position serves as the central point of coordination for patient scheduling, new admissions, visit frequency management, staffing adjustments, and communication among clinical team members.
The Hospice LPN Clinical Scheduler collaborates closely with the Director of Clinical Services and interdisciplinary team to optimize staff productivity, maintain continuity of patient care, and respond quickly to changing patient needs. When operational needs require, the Hospice Scheduler may also provide direct patient care within the scope of an Oklahoma Licensed Practical Nurse.
Duties/ResponsibilitiesClinical Scheduling
- Coordinate daily schedules for Registered Nurses, Licensed Practical Nurses, Hospice Aides, Social Workers, Chaplains, Bereavement Coordinators, Volunteers, and other assigned clinical staff.
- Balance staff caseloads based on geography, productivity, patient acuity, and staffing resources.
- Adjust schedules throughout the day to accommodate admissions, deaths, hospitalizations, call-offs, emergencies, and urgent patient needs.
- Monitor clinician productivity and scheduling efficiency.
Patient Care Coordination
- Coordinate scheduling for new hospice admissions.
- Monitor visit frequencies to ensure physician orders and plans of care are followed.
- Identify overdue or missed visits and coordinate timely completion.
- Collaborate with clinical leadership regarding changes in patient condition or staffing needs.
- Communicate schedule updates promptly with field staff.
Clinical Documentation
- Utilize Well Sky to maintain accurate patient schedules.
- Review scheduling reports for compliance and completeness.
- Assist with correcting scheduling discrepancies.
- Support regulatory compliance through accurate scheduling documentation.
Team Collaboration
- Work closely with the Director of Clinical Services, Clinical Managers, Intake, Team Coordinators, and interdisciplinary team members.
- Participate in interdisciplinary team meetings as requested.
- Serve as a resource regarding scheduling priorities and operational workflow.
Direct Patient Care
- Provide skilled nursing visits when necessary to support patient care needs or staffing shortages.
- Maintain all required nursing documentation and clinical competencies.
- Deliver care consistent with physician orders, hospice philosophy, and agency policies.
Other Duties
- Assist with special projects and operational initiatives.
- Maintain confidentiality and HIPAA compliance.
- Any and all other duties and responsibilities as assigned.
Required Skills/Abilities:
- Thorough understanding of hospice regulations and interdisciplinary care.
- Strong knowledge of visit frequency requirements.
- Excellent organizational and time management skills.
- Ability to prioritize multiple competing demands.
- Strong critical thinking and decision-making abilities.
- Excellent interpersonal and communication skills.
- Proficient with Microsoft Office and Well Sky.
- Ability to work independently while maintaining close collaboration with the clinical team.
Education, Licenses, Certifications and
Experience:
- Graduate of an accredited Licensed Practical Nursing program desired.
- Current Oklahoma Licensed Practical Nurse (LPN) license in good standing required.
- Current CPR certification.
