- Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
- Full time: two weekends per month, with a weekday off following each weekend worked
- Part time: one weekend per month
- $30 per visit, full time
- $22 per visit, part time
- Health and dental coverage

- Weekly pay every Friday
- Provide skilled nursing visits to patients in their homes under the direction of the RN case manager
- Administer medications, wound care, and other treatments as ordered
- Monitor patient condition and report changes to the RN and physician promptly
- Educate patients and families on their care plan, medications, and home safety
- Document visits thoroughly and in a timely manner
- Build real relationships with the patients you see week after week
- Active LPN license in Oklahoma
- Home health experience is a plus, but not required. We welcome LPNs from hospital, facility, or clinic backgrounds who are ready for a change
- CPR certification
- Comfortable working independently and managing your own schedule
- A genuine desire to provide one-on-one, relationship-based patient care
