LPN – Home Health & Hospice

Why choose between Home Health and Hospice when you can do both? This hybrid LPN role is built for nurses who want the best of both worlds: the rehab and recovery focus of Home Health, paired with the deep, meaningful connections of Hospice care. If variety keeps you engaged and you don't want to be boxed into one specialty, this is the role for you.

We've been an Oklahoma City staple since 1999, locally owned and Veteran and nurse owned, which means we understand nursing from the inside out. Our 4.5 CMS star rating reflects the kind of care that reputation is built on.

• Full-time position
• Two weekends per month
• A weekday off following each weekend you work
• Rotating on-call coverage shared across the team

• $30–$35 per visit, negotiable based on experience
• Weekly pay every Friday

• Deliver skilled nursing care across both Home Health and Hospice patients, gaining exposure to two distinct sides of nursing
• Follow physician-directed plans of care and document visits accurately
• Administer medications and treatments within your scope of practice
• Educate patients and families on care plans, medications, and what to expect
• Communicate changes in patient condition to the care team promptly
• Participate in the team's on-call rotation, providing after-hours support as needed

• Active LPN license in Oklahoma
• Experience in home health, hospice, or a related clinical setting preferred
• Current CPR certification
• Comfortable working independently and managing your own schedule
• Adaptable nurses who enjoy variety and want to build skills across both specialties
