Summary The case manager is responsible for the overall management and direction of assigned clients. Ensure that services provided are consistent with the value and mission of City Rescue Mission Inc. The primary functions of this position include but are not limited to:
Implementation of quality services through the individualized service process for the clients served by this department. Areas of expertise may include implementation and ensuring quality of services for our clients, service gaps analysis, building and maintaining relationships with clients, service agency partners, advocacy, and overall coordination of client services. This position will interact with members of other departments as part of the interdisciplinary team approach to problem-solving and meeting the needs of the clients.

Essential Duties and Responsibilities:

Understanding of services and accommodation for people with disabilities.

Ensure that quality individualized services are provided to assigned clients in accordance with City Rescue Mission program policy and procedure.

Use strategic planning to coordinate services, which produce the desired outcomes for clients that provide solutions to their homelessness including access to appropriate supportive services.

Oversee, develop, and implement agency required Individual Action Plan process and overall service coordination for assigned clients within the department.

Work effectively with staff members from other departments and service agency partners in a positive and productive manner.

Ensure each client’s individual action plan (IAP) is comprehensive, effective and includes supporting documentation.

Ensure that data entry into the Agency’s database is complete and accurate for assigned clients.

Report problems and/or concerns to supervisor.

Oversee and maintain the highest quality of services for persons served in City Rescue Mission and through community-based services provided by the agency.

Observe children, elderly, and other vulnerable populations to screen for abuse and/or neglect and other special needs and provide appropriate referrals and case monitoring.

Report incidents of suspected abuse for appropriate action.

Follow agency policy and procedures.

Travel to various locations as necessary for job-related duties, including but not limited to meeting with clients, attending meetings, and accessing resources.

Perform other duties as assigned by the supervisor and/or President/CEO.

Qualifications:

Bachelor's degree (B.A.) in social work or related field (or) experience in case management and program development with people who are experiencing homeless or in a related human service field

Experience with social service agencies and functions

Experience with federal, state, and local grants

Effective verbal and written communication skills

Effective time management and communication skills

Knowledge and understanding in dealing with diverse populations including people with disabilities

Willingness to manage and maintain a client caseload

Proficient use of computers, basic office software and any other data management system used to track service delivery

Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations

Ability to develop relationships with a wide variety of services stakeholders such as schools, health, and welfare agencies

Knowledge of community resources available in various neighborhoods

Valid Oklahoma Driver’s license and good driving record as evidence by a Motor Vehicle Check Understand philosophy and guiding principle of City Rescue Mission
