Position Title:
Care Management Social Worker (Levels 1-3) - Full Time - Adult (Weekends)
Department:OUMC Care Management
Job Description:A Care Management Social Worker provides coordinated care, psychosocial assessment, crisis counseling, education, and advocacy for patients and families navigating health and social challenges.
Essential Responsibilities:Responsibilities listed in this section are core to the position. Inability to perform these responsibilities may result in disqualification.
- Assess conditions via data & interviews.
- Conduct psychosocial assessment; report findings.
- Provide/supervise services (referral, crisis, education, advocacy).
- Educate families on health, finance, stress.
- Monitor progress; adjust interventions.
- Screen risk; offer counseling, abuse education.
- Liaison between patients, staff, agencies.
- Advocate patient concerns; address ethics.
- Maintain respectful, dignified communication.
- Complete mandated reports.
- Mentor Level I or BSW/MSW when supervised.
- Perform other duties as assigned.
1):
Education:
Master of Social Work from an accredited institution.
Experience:
0‑3 years in healthcare (preferred).
License:
Current LMSW or LCSW under supervision;
Basic Life Support endorsed.
2):
Education:
Master of Social Work from an accredited institution.
Experience:
At least 3 years social work.
License:
Current LCSW;
Basic Life Support endorsed.
3):
Education:
Master of Social Work from an accredited institution.
Experience:
At least 5 years social work.
License:
Current LCSW;
Basic Life Support endorsed.
Knowledge of social work principles, community resources, socioeconomic trends, analytical skills, communication, and interpersonal interaction.
