Job Description:

A Care Management Social Worker provides coordinated care, psychosocial assessment, crisis counseling, education, and advocacy for patients and families navigating health and social challenges.

Essential Responsibilities:

Assess conditions via data & interviews.
Conduct psychosocial assessment; report findings.
Provide/supervise services (referral, crisis, education, advocacy).
Educate families on health, finance, stress.
Monitor progress; adjust interventions.
Screen risk; offer counseling, abuse education.
Liaison between patients, staff, agencies.
Advocate patient concerns; address ethics.
Maintain respectful, dignified communication.
Complete mandated reports.
Mentor Level I or BSW/MSW when supervised.

General Responsibilities:

Perform other duties as assigned.

Minimum Qualifications (Level
1):

Education:

Master of Social Work from an accredited institution.

Experience:

0‑3 years in healthcare (preferred).

License:
Current LMSW or LCSW under supervision;
Basic Life Support endorsed.

Minimum Qualifications (Level
2):

Education:

Master of Social Work from an accredited institution.

Experience:

At least 3 years social work.

License:
Current LCSW;
Basic Life Support endorsed.

Minimum Qualifications (Level
3):

Education:

Master of Social Work from an accredited institution.

Experience:

At least 5 years social work.

License:
Current LCSW;
Basic Life Support endorsed.

Knowledge, Skills and Abilities:

Knowledge of social work principles, community resources, socioeconomic trends, analytical skills, communication, and interpersonal interaction.
