Description

Under close supervision, you will assist with individual and/or group services, which include assessment, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way. You will provide a point of contact and work in partnership with others as part of a multidisciplinary team to provide a set of social service functions that help clients access the resources they need for recovery.

Services are provided in the office or in the community based on selected program.

Responsibilities

Assist with case management and rehabilitation services using a strengths-based, welcoming, recovery-oriented, client-driven approach for an assigned group of clients.

Observe the development of strengths-based comprehensive intake screening, assessment; treatment plans; and modification of treatment plans.

Develop a resource guide and learn to collaborate with community resources to assist with clients' needs.

Participate in multidisciplinary team meetings.

Qualifications

Enrolled in a Bachelor’s Degree in the social sciences field.
Strong commitment to adhere to clients' right to have access to helpful, adequate, competent, and continuous supports and services.
Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.
