Care Coordinator- Children & Youth

Position will serve as the facilitator of the child and family teams, advocate for families in the program, provide linkage with needed community resources, assist in treatment and education to families and provide crisis support and safety planning.

• Adheres to agency policy and procedures, CARF and DMHSAS standards, federal and state confidentiality rules, and any special funding source requirements including Medicaid and HCA regulations.
• Facilitates implementation of the phases and activities of wraparound and service coordination, ensuring the team moves children, youth, and family toward long‑term stability.
• Ensures all team members contribute as active partners in the wraparound process.
• Partners with all professional and natural supports on the family team to promote appropriate and integrated services.
• Respects the integrity and confidentiality of the persons receiving wraparound services.
• Conducts strengths, needs, cultural discoveries and crisis plans for each family; prepares written reports and gives them to family according to the wraparound model.
• Develops a strength‑based, family‑guided wraparound plan for each child and family team based on their individual needs.
• Coordinates and facilitates the wraparound process with the child/youth and family team to keep children/youth in their schools and at home, avoiding out‑of‑home placements.
• Empowers the child, youth and family through modeling, advocacy, coaching, follow‑up, and support; teaches how to effectively advocate within child‑serving agencies such as DHS, Juvenile Justice and the education system.
• Develops and maintains positive relationships with community partners; serves as a liaison between Systems of Care and other community services and groups as directed.
• Participates in community consultation and educational activities; promotes child, youth and family involvement at the individual and community levels.
• Monitors eligibility status and applies for Social Security, Title XIX (Medicaid), QMB and other benefits; assists eligible clients in accessing Health Home Services.
• Maintains up‑to‑date client contact information and financial data.
• Assists clients in finding and accessing community resources needed for independent functioning.
• Assists clients, individually and in groups, with development of independent, community living skills, social skills, interests and leisure activities that encourage community involvement and integration.
• Develops individualized recovery plans focusing on personal and interactive problems, independent living skill needs, and goal setting for clients.
• Maintains strict confidentiality guidelines for consumer and family information.
• Provides a minimum of 85 hours of billable services with consumers per month.
• Utilizes the philosophy of Collaborative/Therapeutic Documentation, completing records with the consumer whenever possible and on the same day of service.
• Seeks supervisory assistance for additional training and skill enhancement as needed.
• Collaborates with other professionals to discuss therapy or treatment, new resources, or techniques and to share information.
• Reports and documents services in accordance with agency policy and procedures, completing HOPE documentation by the end of the next business day.
• Work schedule may include late days (e.g., 10 am–7 pm) to accommodate family schedules.
• Must be able to provide services in community settings and provide client transportation using a personal vehicle or agency‑owned van or small bus.
• Performs other related duties as assigned.

• Required to obtain and maintain DMHSAS CM II Certification.
• Required to complete Therapeutic Options or approved safety training and annual updates.
• May be required to obtain and maintain First Aid and CPR Certifications.
• Must possess a current, valid Oklahoma license and meet HOPE insurance requirements; HOPE will annually obtain a copy of the Motor Vehicle Record from the Department of Public Safety.
