- Provides patient care, ensuring patient safety and comfort, according to the Home Health Aide assignment form.
- Reports and records any changes in condition or behavior of the patient.
- Adapts to changes in workload or patient assignments without interference to patient care.
- Responsible for completion of assignments, maintaining quality work.
- Establishes good rapport with patients and families, respecting patients rights to privacy and confidentiality of information.
- Accurately communicates in writing through utilization of appropriate charting methods, and turns in all paperwork on time.
- Provides support to patients and families in a courteous manner.
- Performs routine duties without physical supervision.
- Accurately communicates patients information to the RN Case Manager, LPN and/or Assistant Director of Clinical Services.
- Assists in maintaining equipment assigned to patient care and reports malfunctioning equipment to appropriate personnel.
- Any and all other duties and responsibilities as assigned.

- Proficient with Microsoft Office Suite or related software.
- High School Diploma, or GED equivalent.
- Current Home Health Aide certification.
- Valid CPR Certification.
- Certified to pass Home Health Aide Competency Test.
- 1 year of Home Health Aide or LTCF experience preferred.
- Ability to speak, hear, and read English.
- Lift up to 20 lbs. from floor to 36, and from 36 to 64. (Occasional)

- Sustained forward bending/sitting. (Occasional)
