A Luminos Hospice aide is integral to the compassionate delivery of end‑of‑life care, providing essential support to patients and their families. Duties include assisting with activities of daily living such as bathing, grooming, and mobility, ensuring patients’ comfort and dignity, monitoring vital signs, communicating with the interdisciplinary care team, maintaining accurate documentation, and offering emotional and spiritual support. The aide follows infection control protocols and participates in continuous education to enhance hospice care practices.
Responsibilities:- Assisting with activities of daily living (ADLs) including bathing, grooming, dressing, oral hygiene, toileting, and incontinence care.
- Providing mobility support: transferring in and out of beds or chairs, assisting ambulation and prescribed exercises.
- Monitoring vital signs and reporting to the nursing or hospice care team; observing changes in patients’ physical or emotional condition.
- Ensuring patient comfort through positioning, pain / symptom management assistance, and symptom monitoring.
- Offering emotional and spiritual support to patients and families.
- Maintaining accurate and timely documentation of care provided and observations.
- Collaborating with the interdisciplinary team (nurses, social workers, chaplains) and communicating effectively with patients, families, and professionals.
- Providing end‑of‑life care with sensitivity and compassion; assisting with post‑mortem care when needed.
- Following proper infection control procedures to maintain safety for patients and caregivers.
- Participating in ongoing training and education; maintaining certification and licensure.
Education: High School Diploma Preferred.
Licensure: Driver’s license; CPR certification; state‑approved Home Health Aide certification.
Experience: Minimum one year as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long‑term care facility, or completion of a Home Health Aide Program. Hospice experience preferred.
Skills: Nursing skills as defined by generally accepted standards of practice; knowledge of state and federal regulations; ability to work independently and make sound decisions; strong attention to detail and organizational habits; excellent verbal and written communication skills; good interpersonal skills.
Transportation: Reliable transportation and valid auto liability insurance.
