This role supports Food as Medicine initiatives under New York’s Section 1115 Medicaid Waiver program, helping members with unmet nutrition and social care needs.
Role OverviewYou will manage high-volume inbound and outbound communications, coordinate referrals, support member assessments, and ensure seamless engagement in Epicured’s nutrition services. This role is ideal for someone detail-oriented, empathetic, and comfortable navigating healthcare services in a compliance-driven environment.
Key Responsibilities- Manage inbound/outbound member communications via Five9 contact center (phone/email)
- Conduct referral intake and eligibility screening per NYS Medicaid guidelines
- Schedule and support completion of non-clinical assessments
- Provide navigation to nutrition services and HRSN supports
- Coordinate referrals across internal teams, CBOs, and SCN partners
- Maintain accurate documentation in SCN platform and internal systems
- Support closed-loop referral processes (tracking, follow-up, confirmation)
- Respond to member inquiries regarding services and billing coordination
- Adhere to Medicaid, HIPAA, and NYS DOH compliance standards
- High school diploma or GED (associate’s or bachelor’s preferred)
- Experience in a virtual contact center (Five9 preferred)
- Strong communication, documentation, and organizational skills
- Ability to manage high-volume interactions in compliance-driven settings
- Bilingual (English/Spanish) or other languages (French, Hindi, Georgian, Cantonese, Mandarin)
- Experience in healthcare, care coordination, or patient navigation
- Familiarity with Medicaid, SDOH, or Section 1115 Waiver initiatives
- Community Health Worker or Licensed Clinical Social Worker qualification
- $20–$22/hour
