Minimum Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy from an accredited educational program.
- Active Occupational Therapy state license required at time of submission and to start the assignment.
- Minimum 1 year of OT experience required.
- Experience in long term care (LTC) or rehabilitation settings is required.
- New graduates considered only if they have Level II fieldwork experience (not solely Level I observation).
- Resume required at time of submission.
- Skills checklist within one year required.
- 0-1 professional reference required at time of submission.
- BLS certification from AHA or ARC preferred.
- Experience supervising OT assistants, rehabilitation aides, or students preferred.
- Shift: 8 hour days, 35 GWW.
- Weekend requirement:
None. - May be required to obtain necessary DME or supplies as part of practice requirements.
- Must document treatment minutes and regulatory information in the electronic documentation system per facility policy.

- Provide thorough evaluations using standardized assessments and evidence-based practice.
- Develop and implement individualized plans of care focused on ADL retraining, therapeutic activities, therapeutic exercises, modalities, and assistive device training.
- Monitor progress, adjust treatments, and communicate outcomes with the interdisciplinary team.
- Participate in patient care conferences, utilization review, and rehabilitation meetings.
- Provide comprehensive discharge summaries and ensure compliance with NBCOT and state practice acts.
