Duties and Responsibilities

  • Reports to the Rehabilitation Manager, Lead Physical Therapist or staff Physical Therapist.
  • Treats residents as directed by the Physical Therapist; contributes to the Physical Therapist’s effectiveness by identifying patient‑care issues and providing information and commentary.
  • Monitors patient progress throughout treatment, prepares and maintains records and reports as required, and ensures adherence to procedural safeguards and confidentiality.
  • Documents patient‑care services by charting in patient and department records.
  • Participates in resident care conferences and weekly rehabilitation meetings.
  • Communicates with supervisor and other interdisciplinary team members regarding resident progress, problems, and plans.
  • Participates in facility in‑service training programs.
  • Records daily treatments and labor in accordance with corporate procedures.
  • Instructs families and staff in maintenance programs as directed by the Physical Therapist.
  • Practices safety, environmental, and infection‑control methods.
  • Familiar with Standard Precautions, Exposure Control Plan, and Fire Drill and Evacuation Procedures.
  • Protects residents from neglect, mistreatment, and abuse.
  • Assists with cleaning and maintenance of the treatment area and department.
  • May be required to work beyond normal duty hours, on weekends, or in other positions temporarily when necessary.
Minimum Qualifications
  • Associate's degree.
  • Current license in the state of practice.
  • Certification, if applicable, and demonstrated competency in identified areas of specialization.
  • Organized and detail-oriented work performance.
  • Strong communication skills and excellent self‑discipline and patience.
  • Genuine caring for and interest in elderly and disabled people in a nursing facility.
  • Compliance with the Residents’ Rights and Facility Policies and Procedures.
