Experience:
1 year of community work experience whether a paid position or volunteer
MUST be bilingual (English & Spanish)
Pay & Benefits
Pay: $19.71/hour
11 Federal Holidays paid out

What You'll Do
Provide community-based services tailored to the needs of high-risk and underserved populations and their families
Build trusting relationships and help identify barriers to accessing health care and community services
Explain program purposes and services in a culturally appropriate manner
Support individuals in understanding health management plans, care plans, and instructions to help them reach goals
Document activities, service plans, and outcomes in accordance with established policies and procedures
Coordinate communication and services between individuals and providers/community partners
Maintain and expand a database of community resources and assist with scheduling and applications for eligible programs
What You Need
Required
High School Diploma or equivalent
1+ year of community work experience (paid or volunteer)
Strong organization, documentation, and communication skills (verbal and written)
Computer skills, including Word, Excel, and database software used to track outputs
Ability to work effectively with culturally diverse individuals and communities
