Role

Occupational Therapist in Home Health, responsible for assessing and evaluating patients' functional status, activities of daily living, fine motor skills, and home environments to develop appropriate care plans.

• Provide occupational therapy services within legal scope and evidence-based practices, collaborating with the healthcare team.
• Evaluate patient function, document conditions for CMS, and consult with physicians on care plans.
• Monitor and report patient responses and condition changes to supervisors and physicians.
• Conduct assessments using diagnostic and prognostic tests to determine functional levels.
• Operate effectively in diverse home settings, ensuring equitable care regardless of patient background.

• Current state licensure in Occupational Therapy.
• Valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
• Preferred: CPR certification, ability to manage multiple tasks, work independently, and strong communication and organizational skills.
