Blackburn with Darwen Council is currently on the lookout for a proficient Social Worker to join their renowned Adults Hospital Team, specifically within the Discharge to Assess (D2A) team based at 10 Duke Street, Blackburn, BB2 1DH
. This temporary role, running Monday to Friday 9 am to 5 pm for 37 hours a week, offers a compelling opportunity to utilise your expertise while earning £30 per hour. If the idea of flexibly contributing to the welfare of others excites you, this is the role for you!

What you will do:
  • – Conduct strengths-based social care assessments within D2A assessment bed settings, honing in on the unique needs of each individual.
  • – Undertake Care Act assessments and reviews, ensuring all processes adhere to regulatory standards.
  • – Facilitate safe and timely discharge planning, coordinating with various healthcare professionals to streamline transitions and promote independence.
  • – Evaluate ongoing care and support needs following acute hospital admissions to ensure comprehensive care plans are developed.
  • – Manage care packages and community-based services, ensuring each plan is tailored to patients’ needs.
  • – Collaborate with nurses, therapists, discharge coordinators, and other MDT professionals to promote seamless care coordination.
  • – Advocate for independence, focusing on enabling individuals to return to their preferred living environments whenever possible.
  • – Conduct risk assessments and contribute to safeguarding enquiries to protect and empower service users.
  • – Work closely with patients, families, carers, and partner agencies, ensuring a holistic approach to care management.
  • – Maintain precise and timely case records and reports, ensuring all documentation reflects person-centred care goals.
