Description:
Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative and medically necessary treatment intervention to patients per requirements. Administers various procedures as part of the rehabilitation plan including ADL retraining and therapeutic activities. Assesses patient progress and adjusts treatments accordingly in the clinical record.
Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation. Provides consultation and counseling to patients, families, caregivers, and other service providers. Effectively communicates with supervisors and other health team members regarding patient progress. Documents all required information in the electronic documentation system and maintains patient records. Accurately records treatment minutes for services provided according to requirements. Participates in required meetings including patient care conferences and rehabilitation meetings.
Engages in facility and company required trainings, in-services, and conferences. Able to obtain necessary DME or supplies per requirements. Supervises and supports occupational therapy assistants, rehabilitation aides, and students. Adheres to practice acts, codes of conduct, and ethics during professional practice as an Occupational Therapist.
