The Care Coordinator is a field-based position that plays a critical role in delivering exceptional patient care and ensuring seamless coordination between our clinical team and our contracted long-term care and skilled nursing facilities. Serving as a liaison between facility staff and our providers, the Care Coordinator supports the delivery of timely, efficient and patient-centered care through effective communication, clinical coordination and accurate documentation.
This position requires a highly organized, self-motivated professional who thrives in an autonomous work environment, demonstrates sound judgment and is committed to providing outstanding service to patients, providers and facility partners.
Essential Duties and Responsibilities Coordinate provider schedules and patient visits to ensure efficient care delivery and optimal coverage.
Serve as a primary point of contact between contracted facilities and the clinical care team, ensuring timely communication regarding patients’ needs, treatment plans and follow-up care.
Maintain accurate, complete and timely patient documentation while ensuring compliance with company policies, HIPAA and applicable regulatory standards.
Collaborate with providers and facility staff to promote continuity of care and achieve positive patient outcomes.
Monitor and manage medical supply and equipment inventory to ensure clinical readiness.
Assist with onboarding and training of clinical staff on workflows, documentation standards and best practices.
Identify opportunities to improve clinical processes, communication and operational efficiency.
Regional travel throughout your assigned area to provide support to contracted facilities as needed.
Perform other duties as assigned to support clinical operations and patient care.
Required Qualifications Minimum of one (1) year of experience in long-term care/skilled nursing environment.
Familiarity with long-term care and skilled nursing facility operations.
Excellent verbal and written communication skills with the ability to build positive working relationships with providers, facility staff, patients and families.
Exceptional organizational and time-management skills with the ability to prioritize multiple responsibilities in a fast-paced environment.
Ability to work independently with minimal supervision while exercising sound judgment and accountability.
Working knowledge of HIPAA regulations and healthcare compliance standards.
Proficiency with electronic medical records (EMR) and standard computer applications.
Valid driver's license, reliable transportation and the ability to travel extensively throughout your assigned region.
