A community health worker is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Experience1-3 years CHW experience or equivalent
Certification/RegistrationMercy provided Community Health Worker training certificate within 6-12 months
- Basic Life Support
- Strong relationship and collaboration skills.
- Excellent communication skills.
- Ability to work independently.
- Compassion for poor and underserved members of the community.
- Must be proficient with Microsoft Applications and be able to document in an EMR.
- Must have access to reliable transportation.
Preferred Qualifications Licensure Experience Certification /Registration Other Skills and Knowledge Additional Language Skills As Applicable To Community.
