As a Care Coordinator you will empower, uplift and help others heal from the many forms of mental illness, trauma or addiction. Those who have recently experienced crisis and/or hospitalization, may need additional guidance as they maneuver multiple systems, engagement with new treatment team members and adjust to changes outside of the clinical setting.

To meet the increasing needs of our communities it is imperative that all team members understand and successfully reach and maintain the expected performance standard levels. In order to achieve total and sustainable success each person must to do their part. Lighthouse will do its part in providing any and all necessary tools and training required to help team members be successful.

DEFINITION:

Under immediate to general supervision, provides targeted support services, including case management, rehabilitation and wellness services, a system of care that is child centered with the needs of the child and family dictating the types and mix of services provided and to assist in seamless engagement into outpatient services and aftercare to prevent further out-of-home or out-of-school placement. To be effective, the Care Coordinator may work with the identified consumer and family in their homes, schools and communities, incorporating the family and other social supports in treatment and be focused on the strengths as well as the consumers preferences and culture.

This position is supervised by the respective Director of Outpatient Services.

As a WRAP Care Coordinator, you are more than someone who connects families to services—you are the person who helps bring the right people together, keeps the team moving forward, and helps families build a plan that reflects their strengths, needs, culture, and goals. WRAP is built around the belief that families should not have to navigate complex systems alone, and the Care Coordinator plays a critical role in organizing the Child and Family Team, coordinating services and natural supports, and helping everyone stay focused on what matters most to the family.

Lighthouse is looking for someone who is energetic, proactive, organized, and willing to take initiative. This role requires Dedication—including flexibility to work outside normal business hours when needed, travel to schools and community settings, conduct home visits, and meet families where they are. We are looking for a self-starter who can build relationships, overcome barriers, bring people together, and help families develop the confidence and support system they need to succeed long after formal services end.

EDUCATION AND EXPERIENCE:

A Bachelor or Master Degree in Education, Psychology, Social Work, Sociology, Criminal Justice or similar behavioral health field approved by ODMHSAS; orA current license as a registered nurse in Oklahoma with behavioral health experience; orA Bachelor’s or Master’s degree in any field with 6 months of behavioral health experience; or60 college credit hours in any field and 12 months of behavioral health experience;

orA high school diploma or equivalent and a total of 36 months of direct, documented experience working with persons who live with mental illness and/or substance use issues.

Applicants must be able to provide documented proof from previous employers to justify behavioral health experience.

One year of work experience with juveniles in the mental health or social service field preferred.
