Position: Choosing Childbirth Family Advocate - Choctaw County

The Family Advocate for the Choosing Childbirth Program is dedicated to supporting participants and their families throughout their pregnancy and childbirth journey. This role involves connecting families to essential community resources, offering guidance, and ensuring they receive the necessary support for a positive experience and overall family well‑being. The Family Advocate will play a key role in navigating resources, providing emotional support, and fostering strong community connections.

Duties

and Responsibilities
  • Identify and connect participants, their children, and families with appropriate community resources, including healthcare, social support programs, and educational opportunities.
  • Assist families in accessing services such as prenatal care, childcare, financial assistance, housing, and food support.
  • Serve as a primary point of contact for participants, offering personalized support and advocacy to address individual needs and challenges.
  • Provide guidance on navigating healthcare systems, insurance, and other bureaucratic processes to ensure families receive the necessary services.
  • Educate participants and their families on available resources and how to access them.
  • Provide information on topics related to childbirth, parenting, and family health, ensuring families are well‑informed and empowered.
  • Develop and maintain comprehensive case plans for families, tracking progress and addressing barriers to accessing resources and services.
  • Build and maintain relationships with local community organizations, service providers, and other stakeholders to enhance resource availability and coordination.
  • Represent the Choosing Childbirth Program at community meetings and events to promote the program and strengthen community partnerships.
  • Maintain accurate and confidential records of participant interactions, resource linkage, and case management activities.
  • Participate in team meetings, contribute to program development, and offer feedback for continuous improvement.
  • Other duties as assigned.
Supervisory Relationship

The CCB Family Advocate works under the supervision of the CCB Program Director. This position has no supervisory responsibilities.

Qualifications
  • Degree in Social Work, Human Services, Public Health, or a related field preferred. Experience will be considered in lieu of a degree.
  • Previous experience in advocacy, case management, or community support roles preferred.
  • Strong knowledge of community resources, social services, and support programs.
  • Excellent communication, interpersonal, and organizational skills, with a commitment to serving diverse populations.
  • Ability to work independently, manage multiple cases, and solve problems effectively.
  • Proficiency in Microsoft Office Suite and familiarity with case management software.
  • Ability and willingness to work at times other than regular office hours, including some evenings and weekends.
  • Mental and Physical Demands / Environmental Factors
  • Constantly must be able to sit/stand in a stationary position 50% of the time, stoop, bend; walk; talk; hear; use hands to grip, type; reach with hands and arms; occasionally required to stand or walk on uneven surfaces; climb stairs.
  • Frequently must be able to move about inside the office to access file cabinets, office machinery, etc.
  • Frequently must operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
  • Frequently must communicate with staff and others who have inquiries about the program. Must be able to exchange accurate information in these situations.
  • Occasionally must move/carry items weighing up to 30 pounds.
