ScheduleShift: Monday‑Friday 7 a.m. – 3 p.m. with alternating weekends.
Supervision: RN provides guidance to team members by coordinating use of resources and supporting resolution of issues escalated by LPNs on shift.

Responsibilities- Performs clinical assessments for the identification, evaluation, coordination and management of members’ needs, including physical and behavioral health, social services and long‑term services.
- Identifies members for high‑risk complications and coordinates care in conjunction with the member and health care team.
- Performs assigned waived testing in the laboratory to provide data for diagnosis, treatment, and prevention of diseases; receives specimens; stocks supplies; troubleshoots instrument problems; logs corrective actions; ensures quality control; and trains new employees.
- Draws blood and administers injections as needed.
- Rotates weekends and participates in on‑call duties as defined by the on‑call policy.
- Collaborates with nurse manager to create a teamwork environment that supports improved outcomes and service.
- Supports the facility’s engagement efforts by attending required/mandatory meetings.
- Conducts new admission assessments and completes new admission tasks within the timeframe specified by licensing regulations.
- Assesses and provides first‑aid/appropriate follow‑up for acute medical issues of residents.
- Responds to Medicaid/Level
C incidents and conducts assessments accordingly. - Maintains and manages quality care of residents, working across the care team to ensure residents’ health and wellness needs meet licensing requirements.
- Maintains knowledge of preventative and acute care issues affecting residents; develops treatment care plans and provides education to support residents’ health goals.
- Assists with admission and discharge processes to ensure efficient, high‑quality, patient‑centered care.
- Manages appointment scheduling, shared calendar, paperwork processing, and follow‑ups; documents thoroughly; responds to audits; communicates with all relevant team members and outside medical offices.
- Manages multidisciplinary care, promotes continuity through appropriate admissions, transfers, and discharges.
- Collaborates and communicates with Medical Director and interdisciplinary team regarding residents’ health needs while maintaining HIPAA compliance.
- Completes necessary paperwork and schedules appointments within licensing parameters; reviews and enters new or returning paperwork for all residents.
- Exercises case‑management responsibilities within scope of licensure; develops, monitors, evaluates, and revises treatment care plans to optimize member health care across the continuum.
- Implements treatment care plans by facilitating authorizations/referrals within benefits structure or extra‑contractual arrangements as permissible.
- Manages illnesses or disabilities to ensure cost‑effective, efficient use of health benefits; conducts gap‑in‑care management for quality programs.
- Manages active prescriptions; ensures medication orders are accurate and administered per protocol; completes documentation; monitors for side effects; performs medication audits; educates and supervises med techs and other medication‑management duties.
