Primary Purpose of this Position

You will be responsible for providing direct nursing care to the residents and actively supervise the day‑to‑day nursing activities performed by CNAs and CMAs in accordance with the Oklahoma Nurse Practice Act, and all federal, state, and local regulations and guidelines to maintain and/or enhance each resident’s quality of care and quality of life.

Duties and Responsibilities
  • Utilize personal protective equipment, (PPE), in all appropriate functions and settings in accordance with company Infection Control protocols.
  • Coordinate and cooperate with nursing care and other resident services to ensure the residents total regimen of care is maintained.
  • Participate in the development, maintenance, and implementation of the facility’s quality assurance (QA) program for the nursing department.
  • Participate in surveys/inspections conducted by authorized government agencies as needed.
  • Record any new or changed diet orders and ensure that the appropriate information is disseminated to the dietary department.
  • Chart all communications with the resident’s physicians, other medical professionals assisting with the resident’s care, and the resident’s families.
  • Receive telephone orders and record them appropriately on the Physician’s Order Form and transcribe physician’s orders to the resident’s medical records.
  • Assist with the preparation and planning for the nursing portion of resident’s discharge plans and conduct periodic reviews of discharge care plans and make updates, as required. Enter appropriate information concerning unauthorized discharges in the resident’s medical records.
  • Take and record vital signs in the resident’s charts, among other personal care tasks required by the residents, as needed.
  • Ensure that all medical charting and documentation is complete, and file required forms/charts upon admission, transfer, and discharge.
  • Conduct routine checks of all resident’s care plans to ensure that CNAs and CMAs are conducting those care plans as outlined by the physicians and evaluate the effectiveness of the plan and the emotional and physical status of the residents.
  • Complete accident/incident reports, as necessary and ensure that the resident’s physicians are notified of any change in the resident’s conditions.
  • Prepare and administer medications after first verifying resident’s information in accordance with the 5 Rights of Medication Administration, if/when required. Notify the Director of Nursing, (DON), of any discrepancies with medications, errors with dispensing medication, and/or adverse reactions to medications.
  • Deliver and maintain a high level of resident care and comfort by ensuring a timely response to all call lights.
  • Administer professional services such as catherization, tube feedings and suction, as well as apply and change bandages, packs, colostomy and drainage bags; take blood, conduct range of motion exercises, and care for the dead/dying, etc.
  • Practice good infection control techniques in all aspects of resident care.
  • Communicate effectively with the resident’s, their families, other staff members, and management.
  • Maintain confidentiality of residents and the long‑term community records and information.
  • Protect residents from neglect, abuse, and/or mistreatment, and report any instances of this to the DON immediately.
Qualifications
  • Must be a Licensed Practical Nurse (LPN) in good standing and currently licensed by the state of Oklahoma.
  • Must be at least 18 years of age and be able to pass a background check.
  • Minimum 1 year experience in a Skilled Nursing Facility or Long‑Term Care environment is required.
  • Diploma from an accredited School of Nursing is required.
  • Ability to read, write, and follow oral and written directions at a level necessary to accomplish the job is required.
  • Must be able to relate positively and favorably to residents and families and work cooperatively with other associates.
  • Must be capable of maintaining regular attendance.
