Job purpose

Provides professional, patient-centered dialysis nursing care for the comfort and well-being of patients with end-stage renal disease (ESRD) and acute renal conditions. The PRN Dialysis Nurse administers prescribed medications, monitors patient vital signs, assesses dialysis access sites, and delivers hemodialysis and/or peritoneal dialysis treatments in accordance with physician orders and established protocols. Observes and maintains records on patient care, condition, reaction, and progress.

Notifies the Nurse Manager and physician of any change in patient condition. This position operates on an as-needed (PRN) basis and is considered safety sensitive by Avem Health Partners standards.

Essential functions — include but not limited to Patient care & dialysis treatment

Understands basic renal function and the consequences of renal failure; observes and documents patient responses to renal failure and its treatment.

Performs thorough patient assessments prior to, during, and following dialysis treatments; documents findings accurately and timely.

Initiates, monitors, and terminates hemodialysis treatments in accordance with prescribed dialysis orders and facility protocols.

Recognizes changes in patient condition during dialysis and makes necessary adjustments to therapy; notifies the physician with appropriate clinical information and implements physician orders.

Manages vascular access sites, including cannulation of AV fistulas and grafts, and care of tunneled catheters; monitors for and reports infiltrations or access complications per protocol.

Administers prescribed medications, including intravenous medications and erythropoiesis-stimulating agents (ESAs), according to established facility policies and regulations.

Distributes and documents medications, including controlled substances, in accordance with established standards and regulations.

Participates in anemia management and ensures appropriate action on critical lab values.

Ensures patient discharge management is accurate and complete, including post-treatment assessment and documentation.

Responds promptly and effectively to emergency situations and initiates appropriate interventions.

Performs general patient care duties as assigned.

Assessment, planning & documentation

Contributes to the assessment of the patient and participates in the development and modification of the individualized plan of care.

Evaluates and regularly re-evaluates nursing needs; initiates, develops, implements, and revises the patient's care plan as needed.

Ensures that all required documentation is complete and in compliance with applicable regulations, standards, and facility policies, including accurate data entry.

Maintains confidentiality of patient and employee information at all times.

Completes incident reports accurately and submits to the Nurse Manager; assists with identification of trends as applicable.

Safety, infection control & quality

Ensures all safety and infection control practices are followed, including proper hand hygiene, PPE use, and dialysis machine disinfection protocols.

Assists with cleaning, disinfecting, and maintaining equipment as needed.

Notifies supervisor when supplies need to be ordered or replenished.

Follows policy and procedure to ensure supplies are checked routinely and not expired.

Maintains a clean and safe patient care environment and reports equipment or facility concerns to the appropriate supervisor.

Participates in Quality Assessment Performance Improvement (QAPI) activities to support and improve patient care outcomes.

Follows guidelines and goals in accordance with ESRD Network Program requirements and facility governing body standards.

Communication & teamwork

Communicates effectively with all members of the healthcare team, including physicians, nurse practitioners, dietitians, social workers, and clinical staff.

Provides clinical supervision to CNAs and Patient Care Technicians (PCTs/Hemodialysis Technicians) as appropriate.

Delegates tasks that may safely be performed by others, consistent with educational preparation and that do not conflict with the Oklahoma Nursing Practice Act.

Participates in interdisciplinary care conferences and staff meetings as requested.

Performs other duties as assigned within scope of practice.
