The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. The role includes

• Providing services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.
• Instructing the patient’s family in the care of the patient and maintaining a healthy environment for the patient.
• Observing, recording, and reporting to the supervising nurse and/or physician the patient’s response to treatment, and changes in the patient's condition daily.
• Collaborating closely with physicians and RNs, documenting care accurately and promptly, and may not alter the plan of care.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification.
• Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
