In Home Healthcare LPN - Low Acuity; Weekend Shifts

Licensed Practical / Vocational Nurse (LPN/LVN) position providing skilled nursing care in a home setting. The role focuses on patient assessment, medication administration, and coordination of care.

  Feeding tube & medication administration (training available)

• One-to-one patient care in a home environment.
• Execute physician‑prescribed plans of care and document care provided accurately in the system of record.
• Administer prescribed medication, treatments, and therapies.
• Perform patient assessments and coordinate care.
• Promote health, teach, and train family members.

• Active, unencumbered LPN/LVN license in the state of practice; compact licenses must be transferred within 90 days.
• Current CPR certification (hands‑on component); assistance available after hire.
• TB skin test current within last 12 months.
• Six months of hands‑on nursing experience preferred but not required.
• Must have reliable transportation.
