In Home Healthcare LPN - Low Acuity Weekdays Shifts

Position Summary

We are hiring Licensed Practical / Vocational Nurses (LPN/LVN) to provide skilled nursing care in the home setting for children and adults. The role involves delivering compassionate, client‑centered care and collaborating with the interdisciplinary team to achieve outstanding clinical outcomes.

Monday‑Friday, 8:00 a.m. – 4:00 p.m.

Holdenville, OK

• Provide one‑to‑one patient care in a home environment.
• Execute physicians‑prescribed plans of care and maintain compliant documentation in the system of record.
• Administer prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordinate care with the interdisciplinary team.
• Promote health, teach and train family members.

• Active, unencumbered LPN/LVN license in the state of practice.
• Compact licenses transferred to state of residence within 90 days.
• Current CPR certification (hands‑on component).
• TB skin test performed within the last 12 months.
• Minimum six months of hands‑on nursing experience preferred.
• Reliable transportation required.
