• Active, unencumbered LPN/LVN license in the state of practice
• Compact licenses transferred to your state of residence within 90 days (if applicable)
• Current CPR certification with hands‑on component
• Recent TB skin test (within last 12 months)
• Preferred: 6 months of hands‑on nursing experience (not required)
• Reliable transportation
• State‑specific requirements as applicable

• One‑to‑one patient care in a home environment
• Execute physicians’ prescribed plan of care and document compliant records in the system of record
• Administer prescribed medication, treatments, and therapies
• Conduct patient assessments and coordinate care
• Promote health, teach, and train family members
