Enter Job Description...The Licensed Practical Nurse (LPN) provides direct patient care and nursing support within the outpatient clinical setting of the Anadarko Indian Health Center. The LPN works under the supervision of a Registered Nurse or healthcare provider to deliver quality, patient-centered care, perform clinical procedures, administer medications, and support health promotion activities for Native American patients and the surrounding community.
Key Responsibilities Patient Care- Obtain and document patient medical histories, chief complaints, and vital signs.
- Prepare patients for examinations, treatments, and diagnostic procedures.
- Assist physicians, nurse practitioners, physician assistants, and registered nurses during patient evaluations and procedures.
- Monitor patient conditions and report significant changes to the supervising provider or RN.
- Provide compassionate, culturally sensitive care to diverse patient populations.
- Administer medications, immunizations, injections, and treatments as prescribed and within the scope of practice.
- Monitor patients for adverse reactions and medication effectiveness.
- Maintain accurate medication administration records.
- Perform specimen collection, venipuncture, point-of-care testing, wound care, dressing changes, and other nursing procedures.
- Assist with chronic disease management clinics, including diabetes, hypertension, and preventive health programs.
- Support women's health, pediatric, family practice, and specialty clinic services.
- Prepare and maintain examination rooms and clinical equipment.
- Educate patients and families regarding medications, treatment plans, disease prevention, and healthy lifestyle practices.
- Reinforce provider instructions and discharge plans.
- Promote preventive healthcare services and community wellness initiatives.
- Accurately document patient assessments, treatments, procedures, and education in the Electronic Health Record (EHR).
- Maintain confidentiality and comply with HIPAA, Indian Health Service (Client), and facility policies.
- Review and update patient information as required.
- Adhere to infection prevention and control protocols.
- Ensure proper handling and disposal of biohazardous materials.
- Maintain a safe, clean, and organized clinical environment.
- Follow OSHA, CDC, and Client guidelines.
- Collaborate with physicians, nurses, pharmacists, behavioral health staff, and other healthcare professionals.
- Participate in quality improvement initiatives and staff meetings.
- Assist with care coordination, referrals, and follow-up services.
- Graduate of an accredited Practical Nursing Program.
- Current, unrestricted Licensed Practical Nurse (LPN) license in any U.S. state or territory.
- Current Basic Life Support (BLS) certification through the American Heart Association.
- Minimum one (2) year of nursing experience preferred.
- Experience in outpatient, ambulatory care, family practice, primary care, tribal health, or community health settings preferred.
- Experience working in Indian Health Service (Client), Tribal Health, or Federally Qualified Health Center (FQHC) environments.
- Knowledge of chronic disease management programs.
- Experience with Electronic Health Record (EHR) systems.
- Strong patient education and communication skills.
- Familiarity with culturally competent healthcare practices.
- Knowledge of nursing principles, practices, and procedures.
- Ability to assess patient needs and prioritize care.
- Strong interpersonal and communication skills.
- Ability to work effectively in a multidisciplinary healthcare team.
- Proficiency in clinical documentation and EHR systems.
- Commitment to patient safety, confidentiality, and quality care.
- Frequent standing, walking, bending, lifting, and reaching.
- Ability to lift and move up to 50 pounds with assistance when necessary.
- Ability to perform repetitive hand movements and operate clinical equipment.
- Visual and auditory acuity sufficient to perform nursing duties safely.
- Outpatient ambulatory healthcare setting.
- Exposure to infectious diseases, bodily fluids, and other healthcare-related hazards.
- Requires adherence to infection control and safety standards.
